I understand that HCF collects and uses my personal information provided in this entry form to conduct the HCF promotion, including to determine my eligibility for the HCF promotion. By providing HCF with my name and contact details, I give HCF permission to contact me about the HCF promotion and publish my name on its website if I am one of the winners. If I do not provide this information, I understand that I will not be eligible to enter the promotion or win one of the prizes.

Enter competition at https://enter.hcfteamhealthscore.com.au. Total prize value is $14,974. Prize pool consists of 1x Major Prize and 14x Minor Prizes. 1x Major Prize is 4x reserved seating tickets to the Super Netball Grand Final and to the Grand Final Lunch; if the winner resides outside of Victoria, 4x economy flights to Melbourne, Victoria; and 2x Twin Share accommodation for 2 nights at a hotel. 7x First Minor Prizes are 7x Corporate hospitality Super Netball game family passes and 7x Second Minor Prizes merchandise vouchers for a Super Netball team. Each Minor Prize is for the netball team nominated on the winner's Entry Form. One of each First Minor Prize and one of each Second Minor Prize available per netball team. Competition commences at 8.00am AEDT 4 April 2025 and closes 11.59pm AEDT 13 July 2025. Winners will be selected for the First Minor Prizes at 10.00am AEST 12 May 2025 by manual draw, for the Second Minor Prizes at 10.00am AEST 16 June 2025 by manual draw, and for the Major Prize at 10.00am AEST 14 July 2025 by highest score (or manual draw where there are equal highest). Winners will be notified by email and telephone within 4 days of the relevant draw. Winners' names will be published at https://www.hcf.com.au/netball within 24 hours of the relevant draw.
